You may download the NCSFRI Scholarship Application Form in PDF format by clicking here. The completed form and the following items must be submitted together and postmarked by March 31 or your application will  be denied.

• You must obtain a sealed letter of recommendation from your fire chief or from a community member.
• You must obtain a sealed high school or college transcript.

The award will be presented at the annual North Carolina State Fireman's Association Banquet. The applicant or applicant's parents must be a current member of the North Carolina Society of Fire Rescue Instructors in order to be eligable to recieve the award.
