Complete the online form below, then submit the following items by U. S. Mail, postmarked no later than March 31st or your application will be denied.
 

• A sealed letter of recommendation from your fire chief or from a community member

• A sealed high school or college transcript

 

This award is presented at the annual North Carolina State Fireman's Association Banquet. The applicant or applicant's parents must be a current member of the North Carolina Society of Fire Rescue Instructors in order to be eligible to receive the award.
